UNDATED – Online forms are now available to Bureau, Putnam, and Marshall County Phase 1b eligible residents looking to be vaccinated from the coronavirus. According to the health department, the online form does not confirm a vaccination appointment time, but guarantees that you will be contacted in the future by the BPMHD. If you have already called and given your information to the BPMHD, it is not necessary to fill out a form. Please note that if you have called and given your information to the BPMHD, it is not necessary to keep calling.
